Chronic Conditions Warehouse

The Centers for Medicare and Medicaid’s Chronic Conditions Warehouse (CCW) is one of the largest data warehouses in the world and it is the primary data source for national health services research and analysis, with the goal to significantly improve the quality of care provided to Medicare and Medicaid beneficiaries.

HealthAPT is a Mentor-Protégé Program established through the partnership of NewWave and General Dynamics Information Technology (GDIT) which successfully operates and maintains CCW data.

Implemented in 2005, CCW complies with Section 723 of the Medicare Prescription Drug, Improvement, and Modernization Act of 2003, which required CMS to develop a method for disseminating data to researchers with the goal of improving the quality and cost of care for chronically ill Medicare beneficiaries.

HealthAPT also operates and maintains the Virtual Research Data Center (VRDC) and its associated research tasks within the Office of Enterprise Data and Analytics (OEDA), the Centers for Medicare & Medicaid Innovation (CMMI), and other CMS components.

 The VRDC provides approved users with secure and efficient access to CMS data. Our CCW team maintains the CCW website (www.ccwdata.org), which serves as a unified source of data documentation and analytics.

HealthAPT is responsible for the design, development, and maintenance of enhanced data architecture and infrastructure. HealthAPT also implements sophisticated data migration and integration processes, including automated job scheduling routines, data profiling, and data integrity testing processes. Through HealthAPT, we have integrated new and external data sources while developing related extract methodologies.

Our portfolio of capabilities includes expertise in developing customer-facing virtual assistants or chatbots and leveraging interactive voice response (IVR) services for quick and efficient inquiry resolution.
